Putnam Heritage is dedicated to serving families in Ottawa, Ohio, and Putnam County by providing the best assisted living care for those in need of a little help.

Everything is all-inclusive. There aren’t surprise charges or levels of care like some places. Our monthly fee is never more than $4,650, or just $150 a day including all care, meals, housing and laundry.

We accept both private pay and Medicaid Waiver.

Our History of Heritage

Dr. Charles David founded Putnam Heritage Assisted Living Community in 2004. While in graduate school, Dr. David worked at a group home serving the elderly. This experience left a profound impression on him. He dreamed and prayed about one day creating a unique type of facility that would serve and honor seniors with core principles and values rooted in the belief that no segment of one’s life should be any less meaningful than another.
